This program provides primary care medical services for homeless families, such as exams, immunizations, meds, etc. in a mobile health clinic setting. Also provides mental health referrals, substance abuse referrals, pharmaceutical assistance, dental referrals, and transportation to medical service providers, including the mobile clinic, and case management.
